NOTICE

In order to best meet the needs of our patients, we require 24 hour notice for cancellation of routine physicals. Failure to provide our office with adequate notice will result in a $50 fee. Please note that insurance companies do not reimburse for these fees and that you will be responsible for payment. We appreciate your consideration.
